Major Responsibilities

• Provide nursing care and develop and modify care plans for residents/clients. Attend team meetings.
• Observe, monitor and evaluate condition of clients/residents. Report vital signs, injuries, and general physical and emotional condition. Attend to critical incidents involving clients.
• Measure and record blood pressure, temperature, respirations, pulse, and weight using equipment, such as blood pressure cuff.
• Prepare and administer prescribed medications and treatments, i.e. eye drops and application of ointments & creams. Administer insulin injections. Record quantities administered.
• Inform residents/clients about medications and side effects.
• Ensure medication supplies meet designated inventories. Order medication and medical/nursing supplies. Check orders. Package, label, and store medication and medical supplies.
• Prepare and apply clean dressings and bandages. Clean wounds and lesions, and replace bandages. Remove stitches and clips.
• Respond to medical emergency and render first aid including but not limited to CPR, overdose interventions, etc.
• Orient new residents/clients and staff and familiarize them with surroundings. Notify departments and individuals of admittance. Record and secure personal valuables.
• Prepare resident for transfer or departure, including organization of personal belongings, contacting appropriate organizations (hospital, coroner's office, etc.), and completion of required documentation.
• Schedule appointments with hospitals, clinics, diagnostic services, and health specialists by telephone, arranging transportation and/or escort. Ensure results/hospital records are available to client.
• Assist residents/clients with proper exercise and ambulation techniques to support rehabilitative and activation programs.
• Prepare and maintain documentation i.e. charts, records, and incident reports.
• Accompany physician on rounds, providing information on resident/client general condition and progress, and assisting in examinations, e.g., positioning, dressing and undressing residents/clients.
• Instruct resident/client and family in planning care; offer emotional support; answer questions from resident/client and family. Maintain personal care records.
• Order office supplies, check orders and stock shelves.
• Assist in the operation of weekly clinics in Hostels, including booking of appointments and organizing client files.
• Obtain admission history with clients regarding medical history, immunizations, etc.
• Educate clients and emphasize important health and medical information.
• Refer and advocate with community agencies regarding health and social services. Assist with the organization and implementation of TB screening clinics. Conduct and read TB skin tests and follow up positive reactors. Maintain records.
• Administer TB medication and monitor for side effects.
• Investigate persons who are on medical surveillance or treatment for latent TB.
• Ensure proper storage and handling of vaccines.

Key Qualifications

• Registered and in good standing as a Registered Practical Nurse with the College of Nurses of Ontario.
• Diploma from a recognized college or school of nursing.
• Comprehensive Registered Practical Nurse experience in a shelter, community, or hospital setting with ability to address complex physical, mental health, and addiction needs of marginalized and homeless populations.
• Current First Aid and CPR Certificate.
• Working knowledge/expertise with homeless population, harm reduction and addiction is an asset.
• Ability to provide nursing care to less complex ambulant and non-ambulant residents and clients focusing on restorative, health promotion and palliation.
• Ability to organize and complete daily work activities and provide work direction to staff members as assigned.
• Sensitivity, empathy, and understanding of resident/client behaviour and needs.
• Excellent verbal and written communication skills.
• Ability to work cooperatively with other staff members as part of a nursing team.
• Knowledge of restorative and health promotion interventions to support care plan development and maintenance.
• Knowledge of dementia and mental health diseases to provide care aligning with Hostel Services philosophy and nursing practice standards.
• Physically capable of lifting, turning and transferring residents manually.
• Willingness to participate in in-service education programs.
• Ability to deliver nursing care consistent with standards of College of Nurses, Ministry of Health and Long Term Care Program and applicable legislation and regulation.
• Customer service skills working with clients, service providers, general public and/or staff to address service inquiries, requests, and complaints.

Part-time employees must be available for day, afternoon, and evening shifts including weekends on an as required basis.
