Job Description

The City’s Seniors Services and Long-Term Care Division supports Toronto seniors and residents in long-term care homes to have the healthiest, most fulfilling lives possible through exceptional care and services. Vulnerable individuals who reside in the community are supported through adult day programs, supportive housing services, tenancy support and homemakers and nurses services. Provide nursing care to residents/clients in a Long-Term Care facility and provide work direction to nursing staff. Coordinates services within Long-Term Care Facility and the community for the health care needs of residents/clients.

Major Responsibilities

• Provide Bilingual (French and English) Registered Nurse services
• Provide nursing care and develop and modify care plans for residents/clients
• Attend team meetings
• Observe, monitor and evaluate condition of clients/residents and report vital signs, injuries, and general physical and emotional condition
• Attend to critical incidents involving clients
• Measure and record blood pressure, temperature, respirations, pulse, and weight using equipment
• Prepare and administer prescribed medications and treatments including insulin injections
• Inform residents/clients about medications and side effects
• Ensure medication supplies meet designated inventories; order medication and medical/nursing supplies
• Prepare and apply clean dressings and bandages; clean wounds and lesions; remove stitches and clips
• Administer first aid including C.P.R.
• Orient new residents/clients and staff and familiarize them with surroundings
• Prepare residents for transfer or departure and complete required documentation
• Schedule appointments with hospitals, clinics, diagnostic services, and health specialists
• Feed residents/clients or assist with adaptive eating devices
• Provide personal care including hygiene, bathing, dressing, toileting, ambulation, and transferring using equipment
• Assist residents with exercise and ambulation techniques
• Prepare and maintain documentation such as charts, records, and incident reports
• Accompany physician on rounds providing resident/client condition information and assisting in examinations
• Instruct resident/client and family in planning care; offer emotional support; answer questions
• Change linens, make beds, and sort/bag soiled linen as necessary
• Accompany driver to pick up day care seniors in the community and escort residents/patients on outings
• Order office supplies, check orders and stock shelves
• Assist in the operation of weekly clinics including booking appointments and organizing client files
• Obtain admission history regarding medical history, immunizations, etc.
• Educate clients and emphasize important health and medical information
• Refer and advocate with community agencies regarding health and social services
• Assist with organization and implementation of TB screening clinics
• Conduct and read TB skin tests and follow up positive reactors, maintain records
• Administer TB medication and monitor for side effects
• Investigate persons on medical surveillance or treatment for latent TB
• Inspect physicians' offices for proper storage and handling of vaccines
• Arrange referrals, collect clinical specimens and accompany clients to medical appointments
• Train students

Key Qualifications

• Current registration with the College of Nurses of Ontario in good standing as a Registered Practical Nurse
• Baccalaureate degree in Nursing or an approved Registered Nursing program equivalent
• Experience in long-term care, geriatrics complex continuing care and acute care
• Possession of a valid and current Basic Cardiac Life Support (B.C.L.S) Certification
• Bilingual in English and French in accordance with the French Language Services Act
• Ability to effectively communicate and provide services to residents in both French and English
• Fundamental knowledge and practical application of nursing theory, biological and social sciences to the standard established by provincial nursing legislation
