About The Role

Reporting to the Supervisor Adult Day Programs and under the guidance of the Team Leader, this role assists with the planning, facilitation, evaluation and documentation of social and recreational programs for clients attending the Adult Day Program. Provides and assists clients with activities of daily living (ADLs) to maximize their health, well-being and independence.

What You'll Be Doing

  • Support the delivery of recreational programs that meet client needs in accordance with policies and procedures.
  • Assist with planning programs, including contributing to monthly calendars and completing program plans for group and individual programs.
  • Assist clients with activities of daily living such as serving meals/snacks, medication reminders, washroom, personal care, and mobility assistance.
  • Complete all essential documentation such as Progress Notes, Incident Reports, Attendance Records, Program Plans and Evaluations on time.
  • Report client, caregiver or safety concerns promptly to Team Leader or Supervisor.
  • Assist in maintaining public health standards and inspection documentation including temperature records.
  • Maintain effective communication with staff, volunteers, clients, caregivers and community resources.
  • Ensure services meet Regional customer service standards.
  • Participate in educational opportunities as appropriate.

What We're Looking For

  • Successful completion of secondary school or equivalent.
  • Minimum one year experience in a community Adult Day Program or similar community service agency setting for seniors or related field.
  • Provide a satisfactory Police Vulnerable Sector Check, tuberculosis screening and proof of immunization as required by Long-Term Care Homes Act.
  • Knowledge of gerontology, aging process and dementia programming needs.
  • Demonstrated core competencies of the Region.
  • Strong computer literacy with MS Office applications.
  • Good interpersonal and communication skills to work within a diverse community and empathetically with clients and caregivers.
  • Ability to complete accurate written and electronic documentation.
  • Ability to obtain Food Handling Certificate and Standard First Aid and CPR Level C Certification.
  • Ability to work independently or as part of a team maintaining a safe and respectful environment.
  • Ability to read, write and follow instructions in English.
  • Ability to maintain strict confidentiality and observe, document and report changes in physical/emotional conditions and behaviors.
  • Ability to adapt programming to meet specific client needs including cognitive impairments and physical disabilities.
  • Flexibility to work hours that meet community needs.
